Forms for Proposal Submissions
New/Change/Delete Course Proposal Form (please use for new, changed, or deleted course)
New/Change/Delete Minor Proposal Form (please use for new, changed, or deleted minor)
Change/Delete Program Proposal Form (please use for changed or deleted major, degree, or emphasis -- and for adding a new emphasis to an existing major/degree)
New Program Proposal Form (please use for new major or degree -- please do not use for adding a new emphasis to an existing major/degree)

Special Topics courses
Since the Registrar's Office keeps track of the offering of special topics courses, you do not have to submit special topics proposals to the Faculty Curriculum Committee. However, if you plan to eventually convert a special topics course to a regular course, it might be helpful to examine the Guidelines for New Course Proposals when you plan the course. A special topics course under the same topic should not be offered more than three times within five years. The Registrar's Office will notify the department chair upon the third offering of such a course.
